Patient Payment Plan
In an effort to serve you better and play our role in reducing spiraling healthcare costs, this office is a participant of Patient Payment PlanTM (PPP). We know that sometimes medical treatment can result in a large out-of-pocket expense which can be more than your budget can handle in one lump sum. If this is the case, PPP will allow you to pay your medical services obligation in installment payments that are convenient and affordable after an initial up front payment of 60% of the total bill.
PPP is an automated system, meaning, payments are made through a monthly bank draft funds transfers.
Your payment is automatically drafted from your checking account on the same day each month. You have the choice of having funds deducted on the 4th or the 19th of the month.
Patient Payment Plan charges a $2.50 service charge each draft, in addition to an initial ONE TIME set-up fee of $5.00.
You always have the choice of paying the balance off in full at any time without penalty (only the $2.50 service charge will be added to the balance).

Cancellation Policy
We at Fox Valley Wellness Center understand that a person's schedule can change, necessitating rescheduling of your appointment. We appreciate at least 72 hours notice for new patient appointment cancellations and a 24 hours notice for follow-up appointments. The $150.00 deposit will be kept for any new patient cancellations that are less than 72 hours and half of the office visit fee will be charged for any follow-up visit cancellations less than 24 hours.
